The application listed above needs the following corrections or clarifications.

1. In checking, I did not find an interconnection contract Pending or Approved for Ruby Ranch Internet. These contracts must also contain provisions for Field Connection Point.
2. Applications must be submitted on the forms provided at our Website. These applications may be downloaded, filled out and attached when sent via E-mail to us at the mailbox stated below.
3. Your e-mail stated “This email follows up on Susan Griffeths' telephone call to me of yesterday .Collocation Application” If there are special conditions, etc to this collocation, I have not been notified by anyone.

PLEASE REDATE APPLICATION TO CURRENT DATE BEFORE RESUBMISSION.
