This email follows up on Susan Griffeths' telephone call to me of yesterday.  

Collocation Application

a) Identification of the Qwest Premise. Cross-connect box at the intersection of Ruby Road and Emerald Road, Summit County, Colorado.

b) Floor space requirements, including the number of bays for a cageless Collocation arrangement. None.

c) Power requirements. None.

d) Heat Dissipation. None.

e) Type of Collocation (e.g., Caged Physical, Cageless Physical, Shared, Virtual, etc.). Cross-Connect Collocation.

f) Collocated Equipment and technical equipment specifications. None other than copper cable.

g) Entrance Facility Type. Not applicable.

h) Type and Quantity of Terminations. 200 or 100 terminations. Note that there are presently openings for two 100-pair binding post panels.

i) If desired, an alternate form of Collocation if first choice is not available. Not applicable.
