I understand, I received a message today that you were working with someone on negotiations and you've had a service manager assigned to your account if you are ready to start ordering, but I checked and I don't show one of our questionnaires or anything like that set up or I don't show where you have had contact with any of the other implementation managers. I would be glad to work with you on this. I am at 801-239-4244. Back on July 6 your account was handed off to me by Tony Barone, and on that day we had called all the accounts and had left word, just voice mail, maybe you weren't ready at the time. First of all on the collocation request, when that form is completed, it needs to be emailed to rfsmet@qwest.com and when that is received, you will be notified to set up a call, to make sure that what is on the form is exactly what you would like, and then that process starts. if you aren't going to have any voice, we will need a letter saying that you will not have voice, and thus will not need 911 service and probably operator services, kind of saying that you are not providing voice. I can talk with you about that later. ... When you get a minute why don't you give me a call back. 801-239-4244.
I then reached her on the phone and we went over the places where she wants me to email the DLEC questionnaire, the collocation request form, and the Field Connection Point request form. She also asked me to send her a 911 waiver letter, which I did.
